The United States spends approximately 25 percent of Medicare on the 5% of patients in their final year. Unfortunately, most of this care does not have a significant benefit to the patient’s well-being. The United States is also not alone, with the Netherlands and Switzerland showing similar rates of expenditure.

1Unit’s Goals of Care Conversations program teaches unit staff how to introduce, schedule and conduct a structured Goals of Care conversation with patients and families to elicit their wishes and desires, with the patient’s optimum care and care plan as the central goal.
